Tear troughs are semi-circular creases/depressions which start at the nose and run along the bottom of the lower eyelid just above the cheekbone. These depressions can produce the illusion of darkening around the eyes resulting in ‘dark circles’ or ‘tired eyes.

As we age, the eyelid above and the cheek below begin to sag. However, the depression cannot sag with the rest of the facial skin due to its ligamentous attachment to the bone of the eye socket. This results in the depression deepening and the shadow and dark circle becoming more noticeable.

This is a revolutionary technique invented and perfected by Dr Bong. He formulated this technique with one of the world’s best dermal fillers for this area – Teosyal Redensity II [eye].

A blunt-tip device (called micro-cannula) is used to administer the filler in the tear trough area to soften the appearance of the hollowness, eye bags and dark circles. The Two-Point Eye Lift technique, coupled with the use of the blunt-tipped cannula and Redensity II [eye], help minimise side effects and complications commonly associated with treatment in this area.

90% of our patients do not require any form of an anaesthetic agent at all, and they describe this procedure as less uncomfortable than getting a vaccine or having a blood sample taken from your arm. To provide further relief and alleviate as much pain as possible, we use only premium, industry-leading soft tissue fillers which contain a pre-mixed anaesthetic agent.

While traditionally, soft tissue fillers have always been injected using sharp-tip needles, a new alternative to using needles is using blunt-tipped cannulas. Although there is a role for both injection methods, the cannula offers some excellent benefits to patients.

  1. Sharp needles can pierce veins and arteries beneath the skin. Since blunt cannulas push aside any arteries and veins encountered beneath the skin, there is a reduced risk of bleeding and bruising.
  2. Sharp tip needles use a very thin, short, sharp needle to inject dermal fillers. Multiple injection points are required when using needles, which can increase the risk of hitting a vein. If a vein is hit with a needle, there is a higher chance of bruising after the procedure.
  3. Sharp tip needles may also result in trauma to the skin, causing swelling.

Advantages of blunt-tipped cannulas
Cannulas have a hollow bore, like needles, allowing the filler to pass through into the tissue. However, cannulas have a rounded, blunt end instead of sharp tip-like needles. It is a bit like a long needle, but the tip is blunt. The process involves numbing the skin with ice and injecting a small amount of local anaesthetic beneath the skin with a tiny needle. When the skin is numb, a needle is used to pierce the skin, and then the cannula is threaded into the puncture site, sliding along the natural planes of tissue, providing access to a large area for filler placement.

  1. Less Pain During Treatment – Being injected multiple times with sharp needles can be painful. Still, while it may sound a bit uncomfortable to have a cannula beneath your skin, there is minimal discomfort. Patients often note that they experience a strange “pushing and tugging” feeling, but there is very little pain. Fewer pokes with needles also reduce the amount of pain, even when multiple areas of the face are being treated.
  2. Decreased Risk of Bleeding and Bruising – Sharp needles can pierce veins and arteries beneath the skin. Since blunt cannulas push aside any arteries and veins encountered beneath the skin, there’s a reduced risk of bleeding and bruising.
  3. A Safer Option – When using needles for filler injections, one of the scariest potential complications is accidentally injecting filler into an artery, which can cause tissue death. In extremely rare cases, blindness has occurred. Cannulas do not pierce arteries, offering a much safer option for patients.
  4. Minimal Downtime – Since needle-injected fillers can cause swelling and bruising after the procedure, they often have some social downtime. Since the cannula injection method cuts down on swelling and bruising, there is minimal downtime post-procedure.
  5. Precision – With blunt-tipped cannulas, fillers can be injected more accurately, reducing the amount of filler needed to achieve the desired results.
  6. Greater Flexibility – A much larger surface area can be treated with just a single injection site when using the cannula method, making it possible to get more done with fewer injection points.

The results of the Two-Point Eye Lift technique are noticeable instantly. After the treatment, the treated area may be slightly red. However, you can resume normal activities, drive home, or return to work immediately after the treatment. One of the significant advantages of the products we use at Essence Medical is that the effects are long-lasting but not permanent. Therefore you are always in control of your looks and can change your treatment depending on ageing. The results typically last 9-12 months, although the exact length of time will depend on the structure of your skin and other factors such as lifestyle, age and the degree of perfection you demand. Additional touch-up treatments can be performed at any time after the initial treatment.

• Injection-related reactions: These reactions may include redness at the injection site immediately after the treatment.

• Bruising: Occasionally, bruising can occur. However, the use of blunt-tipped instruments dramatically reduces the risk of bruising.

• Contour irregularities can and do occasionally occur. Massaging can usually smooth out any ‘lumpiness’; however, the filler can be easily and rapidly dissolved if persistent.

• Infection – as with all injections, this procedure risks infection. The syringe is sterile and standard precautions associated with injectable materials are taken.

• Swelling – Unlike generic dermal fillers for tear trough treatment, our choice of premium filler results in only very minimal or no swelling at all.

After your treatment, we will again go through with you any aftercare advice to maximise your treatment benefits and avoid any undesirable side effects. You can resume normal activities, drive home, or return to work immediately after the treatment.

As with any injection, there may be some slight redness immediately afterwards, and camouflage makeup may be applied. However, the results directly after the treatment should not be seen as the final results. This is because the results will gradually improve over the following 2-4 weeks.

For two weeks after the treatment, you are advised to avoid extreme heat (sun bed or sunbathing) or intense cold.
